  • TRANSCRIPTION OF CIVIL CERTIFICATES

  • Registration a foreign birth certificate is based on a literal transfer of the contents of a foreign civil status document (birth certificate) in the Polish Registry. After the transcription is being done, the head of the registry shall issue a complete copy of the Polish birth certificate, which shall contain the note that the same has been made. The transcription (registration) of a foreign document is obligatory If a Polish citizen who´s  document concerns  is applying for civil status registration, or applying for a Polish identity card or PESEL number.

     

     

    An application for transcription of a birth certificate can be submitted personally, by the consul  or through a representative to the head of the civil registry office in Poland. The transcription of the birth certificate is most often made in connection with the birth of a child outside the Republic of Poland, in order to be able to issue a passport document or to conduct a procedure to confirm the possession of Polish citizenship.

    1. Applying for transcription of the birth certificate: You must complete the applications in two copies and sign them during the consular meeting. WARNING! Applications should be completed according to the data that contains the given birth certificate. Any errors in the dates, places or names and surnames in the original document must be corrected before beginning the procedure.

       

    2. The birth certificate issued by the civil registry of the country of birth must be legalized by the Ministry of Foreign Affairs of that country. Venezuelan documents intended for trading in Poland should have apostille. it is required to submit the original birth certificate along with the translation into Polish + one copy. The original will be sent to Poland and it is not returned.

       

       

    3. If the mother of a Venezuelan child is named after her husband, another civil status certificate must be provided to indicate the mother's maiden name (e.g. parental marriage certificate). In this case you should submit the file together with the apostille and the translation into Polish + two copies. Original to view

       

     

    An application for transcription of a marriage certificate can be submitted personally, by the consul  or through a representative to the head of the civil registry office in Poland. The transcription of the marriage certificate is most often made in connection with  the marriage contracted outside the Republic of Poland, in order to be able to issue a passport document with a surname´s change or to conduct a procedure to confirm the possession of Polish citizenship.

    If the information on the names used by the spouses after the marriage and the names used by the children born of the marriage does not appear in the foreign marriage certificate, statements relevant to this may be included in the application. In the event that the request for transcription of the foreign marriage certificate has been presented to the consul, these statements of both spouses may also be filed formally with the same.

     

    1. Applying for transcription of the marriage certificate: You must complete the applications in two copies and sign them during the consular meeting. WARNING! The consular meeting should be attended by both spouses to sign the applications.  Applications should be completed according to the data that contains the given marriage certificate. Any errors in the dates, places or names and surnames in the original document must be corrected before beginning the procedure.

       

    2. The marriage  certificate issued by the civil registry of the country of birth must be legalized by the Ministry of Foreign Affairs of that country. Venezuelan documents intended for trading in Poland should have apostille. it is required to submit the original marriage certificate along with the translation into Polish + one copy. The original will be sent to Poland and it is not returned.

     

     

    To register a foreign birth certificate  of a minor child (under 18) in Poland the following documents shall be submitted:

    1. Applying for transcription of the birth certificate of a minor : You must complete the applications in two copies and sign them during the consular meeting. WARNING! The consular meeting should be attended by both parents to sign the applications . Applications should be completed according to the data that contains the given birth certificate. Any errors in the dates, places or names and surnames in the original document must be corrected before beginning the procedure.

       

    2. The birth certificate issued by the civil registry of the country of birth must be legalized by the Ministry of Foreign Affairs of that country.
